NTSB Preliminary Report: Kent Island, MD

On May 4, 2019, about 1210 eastern daylight time, a Guimbal Cabri G2, N572MD, was destroyed when it impacted the Chesapeake Bay near Kent Island, Maryland. The private pilot and passenger were fatally injured. The helicopter was registered to a corporation and operated by Monumental Helicopters as a Title 14 Code of Federal Regulations Part 91 personal flight. Instrument meteorological conditions prevailed for the flight, and a special flight rules area flight plan was filed for the local flight that originated from Tipton Airport (FME), Fort Meade, Maryland, around 1130.

On May 15, 2019, at 1426 eastern daylight time, a Bell 206 L4 helicopter, N26BB, operated by Zip Aviation, was substantially damaged during a forced landing to the Hudson River while maneuvering for landing at the West 30th Street Heliport (JRA), New York, New York. The commercial pilot was not injured. Visual meteorological conditions prevailed, and no flight plan was filed for the repositioning flight conducted under the provisions of Title 14 Code of Federal Regulations Part 91.

ASU one of three companies to deliver helmet prototypes to U.S. Air Force

Aviation Specialties Unlimited, Inc., (ASU) announced today that they delivered four prototype helmets to the U.S. Air Force at Nellis Air Force Base in Las Vegas, Nevada. ASU was one of only three finalists selected by the AFWERX Fixed Wing Helmet Challenge in November of 2018. More than 110 companies submitted helmet designs initially in 2018. In October of 2018, the field narrowed to 38 companies and lastly to the final three.

The commercial pilot was not injured, and the helicopter sustained substantial damage. NTSB Preliminary Report: St Jacob, IL

On April 20, 2019, at 1930 central daylight time, a Robinson Helicopter Company R66, N766WC, impacted an unmarked powerline and terrain during an aerial application of a field near St Jacob, Illinois. The helicopter sustained substantial damage. The airline transport pilot received serious injuries. The helicopter was registered to and operated by Skyline Helicopters LLC under Title 14 Code of Federal Regulations Part 137 as an aerial application flight that was not operating on a flight plan. Visual meteorological conditions prevailed at the time of the accident. The local flight originated from St Jacob, Illinois at 1915.

NTSB Factual Report: Sula, MT

The helicopter pilot reported that, after an aerial application flight, he set the bucket down in a pre-determined operations zone near a semi-truck loading hopper to be refilled. He added that another truck arrived to top off the semi-truck hopper, which emitted fertilizer dust that caused the pilot concerns for his deteriorating visibility. He advised the ground crew that he was going to move out of the operations zone temporarily.
