NTSB Preliminary Report: Garwood, TX

On June 29, 2019, about 1230 central daylight time, a Robinson R-22 (Beta) helicopter, N7143Y, impacted a wire while maneuvering low-level near Garwood, Texas. The commercial pilot and sole occupant sustained serious injuries. The helicopter sustained substantial damage. The helicopter was registered to Spitzer Helicopter, LLC and was operated by the pilot under the provisions of Title 14 Code of Federal Regulations Part 91 as a visual flight rules crop pollination flight. Day visual meteorological conditions prevailed at the time of the accident, and no flight plan was filed. The flight originated from a private helipad near Garwood, Texas at an unknown time.

Following the introduction of the industry leading DVCS6100 digital intercom system in 2008, Becker Avionics will now also offer the innovative AMU6500. This new generation digital intercom system incorporates several advanced features such as 3D audio technology and Bluetooth compatibility. 3D audio allows for customizable left or right channel separation into crew helmet or headsets for corresponding left or right engine or system(s) malfunction(s). Selectable channel separation of left or right audio radio frequencies for complex missions with multiple radio frequency operations is also available. Enhanced safety and efficiency by reducing crew workload along with improving situational awareness were primary design considerations of the AMU6500 at Becker Avionics.

On June 17, 2019 about 1730 Pacific daylight time, a Robinson R44 helicopter, N7187D, was destroyed when it impacted terrain while maneuvering near Avalon, California, Santa Catalina Island. The pilot was fatally injured. The helicopter was registered to and operated by the pilot as a Title 14 Code of Federal Regulations Part 91 personal flight. Instrument meteorological conditions prevailed within the area and no flight plan was filed. The local flight originated from Long Beach Airport (LGB), Long Beach, California at 1657.

Leonardo’s Falco EVO drone is used to monitor irregular migration during Frontex operation

On the 20th June, Leonardo’s Falco EVO tactical remotely-piloted aerial system played a key role in monitoring a case involving irregular migrants in the Mediterranean sea as part of monitoring activities of Frontex, the European Border and Coast Guard Agency. In an operation launched from the Italian island of Lampedusa, the Falco EVO worked with other Frontex assets to identify a “mothership” trawler as 81 illegal migrants were transferred to smaller boats, a technique increasingly used by criminal organisations. Following the transfer, the Falco EVO’s surveillance payloads kept a close eye on the trawler until an enforcement operation was launched by Italian authorities to seize the boat.

NTSB Preliminary Report: Brainerd, MN

On June 28, 2019, about 0041 central daylight time, an Agusta Spa A109S helicopter, N11NM, impacted terrain while landing at Brainerd Lakes Regional Airport (BRD), Brainerd Lakes, Minnesota. The pilot and flight nurse were fatally injured, the flight paramedic was seriously injured, and the helicopter sustained substantial damage. NTSB Preliminary Report: Parsons, WV

On June 29, 2019, about 1140 eastern daylight time, a Robinson R44 helicopter, N883TY, was substantially damage when it collided with a telephone line and then a river near Parsons, West Virginia. The private pilot and one passenger sustained minor injuries, and a second passenger sustained serious injuries. The helicopter was registered to and operated by Ali Co. under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed at the time of the accident and no flight plan was filed for the local flight that originated about 15 minutes earlier from Elgon, West Virginia.

NTSB Final Report: Orlando, FL

The commercial pilot was conducting a local sightseeing flight in a helicopter with two passengers onboard. He reported that, while he was flying southbound and descending from 1,000 to 600 ft, he heard a "pop" sound and noted a corresponding right yaw, followed by vibration and engine roughness. The pilot looked at the instruments, and the main rotor tachometer was indicating that the rpm had dropped to "0" and that the engine tachometer was indicating between 60% and 70%. The pilot immediately initiated an autorotation to a median, and the helicopter landed hard. The helicopter began rolling forward, and in response, the pilot applied aft cyclic control input, which was contrary to the instructions in the Pilot's Operating Handbook, which contained a caution in the emergency procedures section related to power failures that stated to "not apply aft cyclic during touchdown…to prevent possible blade strike to the" tailboom. The pilot's improper control input resulted in the main rotor blades contacting and substantially damaging the tailboom.
