NTSB Preliminary Report Released: McDougal, AR

On October 2, 2018 about 1330 central daylight time (CDT), a Hughes 369D, N5187S/N454AR, was destroyed when it impacted a utility pole and, subsequently, terrain in McDougal, Arkansas. The helicopter departed from a grass landing zone with two linemen attached for short haul operations in connection with utility line work. The commercial pilot was fatally injured and the two lineman sustained minor injuries. Visual meteorological conditions prevailed for the external load operation that was conducted under the provisions of 14 Code of Federal Regulations Part 133, and no flight plan had been filed.

NTSB Preliminary Report Released: Wichita Falls, TX

On November 2, 2018, about 1130 central daylight time, a Bell 47G-3B-1, N1159W, impacted terrain after experiencing a total loss of engine power near the Wichita Valley Airport (F14), Wichita Falls, Texas. The certified flight instructor (CFI) and airline transport pilot were seriously injured and the helicopter exhibited substantial damage throughout. The helicopter was registered to Treblig LLC and operated by the pilot as a Title 14 Code of Federal Regulations Part 91 instructional flight. Visual meteorological conditions prevailed and no flight plan was filed. The flight originated from F14 at about 1105.

The airline transport rated pilot and two passengers were fatally injured, and the helicopter was destroyed. The helicopter was registered to and operated by W T Byler company, Inc. as a 14 Code of Federal Regulations Part 91 personal fight. The flight departed a private ranch about 2345 en route to San Antonio.

On October 30, 2018, at 1611 eastern daylight time, an Aerospatiale AS355, N372CA, was destroyed when it collided with a wooden utility pole and power lines near Beekmantown, New York. The airline transport pilot and a lineman were fatally injured. The pilot-rated passenger and another lineman sustained serious injuries. The helicopter was registered to and operated by Catalyst Aviation, LLC, Hammonton, New Jersey, and was being operated under the provisions of Title 14 Code of Federal Regulations Part 133 as a rotorcraft external load flight. Visual meteorological conditions prevailed, and no flight plan was filed for the flight that departed from a designated landing zone near Plattsburgh, New York, at 1602.

NTSB Final Report Released: Burnet, TX

The pilot and two passengers had conducted aerial hog hunt operations from a helicopter and were en route to an airport when the engine lost power. The pilot conducted an autorotation to a road; however, the helicopter landed hard, resulting in substantial damage. An examination of the helicopter revealed that the fuel tank was empty, and no residual fuel was found in the system. Since the pilot was en route to an airport where fuel was available, it is likely that he spent too much time airborne and misjudged the amount of fuel needed to reach the destination. The accident is consistent with a fuel exhaustion event.

NTSB Final Report Released: Medford, NJ

The purpose of the flight was to provide an orientation/pleasure flight to the passenger, who was scheduled to perform in a concert on the airport later that evening. Several minutes after takeoff, the pilot reported over the airport UNICOM frequency that he was unable to control engine rpm with throttle inputs. He reported that he could "roll" the twist-grip; however, there was no corresponding change in engine power when he did so.
