NTSB Preliminary Report: Montgomery, NY

On March 30, 2019, about 1230 eastern daylight time, a Bell 222, N222ML, was substantially damaged during a forced landing shortly after takeoff near Montgomery, New York. The private pilot was not injured, while one of the five passengers sustained minor injuries. No flight plan was filed for the local personal flight that was conducted under the provisions of Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed at the time of the accident.

NTSB Preliminary Report: Tampa, FL

On April 4, 2019, about 1416 eastern daylight time, a Robinson R44, N4046J, was substantially damaged during a forced landing to a roadway near Tampa, Florida. The pilot and passenger were not injured. The passenger in a vehicle was fatally injured and the driver incurred minor injuries from flying debris. Visual meteorological conditions prevailed, and no flight plan was filed for the positioning flight, which was destined for Sarasota/Bradenton International Airport (SRQ), Sarasota, Florida. The flight was conducted under the provisions of Title 14 Code of Federal Regulations Part 91.

NTSB Final Report: Spartanburg, SC

The private pilot of the experimental, amateur-built helicopter reported that he returned to the airfield after a local flight and was preparing to land. While flying about 4 ft above ground level at an indicated airspeed between 16 and 24 mph, he felt a shudder, and the helicopter yawed to the right. He straightened the helicopter with antitorque pedal inputs and did not recall hearing any engine noise, although the doors were removed. The next thing that he recalled was lying on the ground, with the helicopter leaking fuel. Examination of the wreckage revealed that the helicopter's main rotor blades were bent and the tailboom was severed. Postaccident examination of the helicopter, which included an engine run, did not reveal any evidence of mechanical malfunctions or failures that would have precluded normal operation. Although the pilot reported 46 total hours of helicopter flight experience, all in the make and model of the accident helicopter, he did not possess a rotorcraft rating at the time of the accident.

NTSB Final Report: Bella Vista, CA

The commercial pilot reported that, while making a left descending turn in the helicopter about 75 to 100 ft above a pond to set up for landing to a field, he heard three "pops," followed by the low rotor horn sounding and the caution light illuminating. The pilot assumed that the engine had lost power, and he immediately and abruptly lowered the collective, pulled aft on the cyclic, and rolled the throttle to full open. To avoid obstructions, he steepened the right turn. Realizing that the rotor rpm was below 50% and that the helicopter would not reach the field, he subsequently landed the helicopter on the pond; it then sank. The pilot was unsure if the engine was running or not

NTSB Final Report: Dennis, WV

The commercial pilot was conducting long line operations in the helicopter, and he was flying it in an aft or left attitude with a higher nose-up attitude than normal flight to compensate for the load's drag. He then transitioned the helicopter to a 100-ft hover over a landing zone, and shortly thereafter, the helicopter experienced a total loss of engine power. Subsequently, the pilot initiated an autorotation, and during the landing, a main rotor blade contacted and severed the tailboom.

NTSB Factual Report: Keene, NH

The helicopter pilot reported that while enroute, about 2 miles northeast from his destination airport, about 500 feet above ground level and 60 knots, he heard a loud "thud" from a possible bird strike and the upper right windscreen/canopy shattered, with pieces going into the main rotor disc and cabin. He continued to the airport and landed without further incident.
