HQ Aviation gains approval as RR300 Authorized Service Center

Denham, UK - HQ Aviation, the very successful British helicopter training school and maintenance centre has expanded its list of approvals through its appointment as a Rolls-Royce RR300 Authorized Service Center. Engine manufacturer Rolls-Royce today at Heli-Expo 2019 officially confirmed HQ Aviation has joined the RR300 Service Center network, which offers global, approved maintenance support to RR300 powered Robinson R66 operators worldwide. HQ Aviation was assessed by H+S Aviation, who have been given delegated rights by Rolls-Royce to ensure compliance with the engine manufacturer's requirements.

NTSB Factual Report: Keene, NH

The helicopter pilot reported that while enroute, about 2 miles northeast from his destination airport, about 500 feet above ground level and 60 knots, he heard a loud "thud" from a possible bird strike and the upper right windscreen/canopy shattered, with pieces going into the main rotor disc and cabin. He continued to the airport and landed without further incident.

NTSB Final Report: Chalmers, IN

On March 14, 2017, at 1546 eastern daylight time, an MD Helicopters 369FF, N530KD, impacted terrain during a power line construction flight near Chalmers, Indiana. The commercial pilot was fatally injured, and the helicopter was destroyed. The helicopter was registered to a private individual and operated by Rogers Helicopters, Inc., under the provisions of Title 14 Code of Federal Regulations Part 133 as an external load operation. Visual meteorological conditions prevailed at the time of the accident, and no flight plan had been filed. The helicopter departed from a staging area near the accident site about 1530 for the local flight.

NTSB Final Report: Dimmitt, TX

The private pilot, who did not hold a rotorcraft rating, departed in the helicopter on a routine flight to check and observe crop fields, business facilities, and other business-related assets. A search was initiated when the pilot did not return from the flight, and the accident site was located in a field the following morning. There were no witnesses to the accident. Data obtained from an onboard GPS showed the helicopter making a series of turns at a low altitude above the field in which the wreckage was located. Examination of the helicopter did not reveal any preimpact mechanical malfunctions and there was no damage to nearby obstructions to suggest an in-flight collision.

NTSB Final Report: Entiat, WA

The pilot reported that, while maneuvering the helicopter at low altitude for an agricultural application flight, an unsecured clipboard that was on the seat next to him fell and became lodged under the antitorque pedals. Unable to free the clipboard, the pilot chose to make a precautionary landing. About 5 to10 ft above ground level, the pilot lost tail rotor authority, and the helicopter began to rotate to the right. After about two rotations, he was able to land the helicopter. During the touchdown, the stinger folded into the tail rotor.

The helicopter had just touched down, so the flight instructor secured the cockpit and both he and the student egressed the helicopter with the engine still running. About 5 minutes later, the engine stopped.

NTSB Preliminary Report: Forks, WA

On March 8, 2019, about 0705 Pacific standard time, a Bell UH-1B helicopter, N64RA, collided with mountainous terrain near Forks, Washington. The commercial pilot was fatally injured. The helicopter sustained substantial damage to the fuselage, tail boom, and drivetrain. The helicopter was registered to and operated by Iron Eagle Helicopters under provisions of Title 14 Code of Federal Regulations Part 133, as an external load operation. Visual meteorological conditions prevailed, and no flight plan was filed for the off airport local flight. The flight originated from a remote location and flew to the contracted logging site about 1/2 miles to the south-west.

NTSB Final Report: Groom, TX

On October 24, 2017, about 1540 central daylight time, a Bell 206B helicopter, N60EA, sustained substantial damage during a forced landing after a total loss of engine power while on approach to a private landing zone near Groom, Texas. The pilot was not injured. The helicopter was owned and operated by Plains Aerial Leasing, Inc as a Title 14 Code of Federal Regulation Part 137 agricultural aerial application flight. Visual meteorological conditions existed near the accident site at the time of the flight, and a flight plan had not been filed. The flight departed from an off-airport landing zone about 1515.
