NTSB Report Released: Santa Ana, CA

On July 26, 2017, about 2000 Pacific daylight time, a Robinson Helicopter R44, N7535G, was substantially damaged following a loss of control during takeoff at John Wayne-Orange County Airport (SNA), Santa Ana, California. The commercial pilot, the sole occupant, was not injured. The airplane was registered to and operated by RS Aviation Services as a 14 Code of Federal Regulations Part 135 flight. Visual meteorological conditions prevailed for the flight which operated on a company flight plan. The flight originated from SNA with an intended destination to Burbank, California.

NTSB Report Released: Ireland, IN

On July 8, 2017, about 1040 eastern daylight time, a Robinson R44 helicopter, N7224U, impacted the ground following a loss of control while conducting an aerial application flight near Ireland, Indiana. Following the impact with the ground, a fire erupted and the helicopter was destroyed. The pilot received serious injuries. The aircraft was registered to and operated by JR Aviation , Inc., under the provisions of 14 Code of Federal Regulations Part 137 as an aerial application flight. Visual meteorological conditions prevailed for the flight, which was not operated on a flight plan. The local flight originated from an unconfirmed location at an unconfirmed time.

NTSB Report Released: Ontario, CA

On July 19, 2017, about 1010 Pacific daylight time, an Enstrom F28-C rotorcraft, N5697B, experienced a partial loss of engine power and landed hard in a dirt pasture about 1/2-mile northeast of the Chino Airport (CNO), Ontario, California. The student pilot, sole occupant, was seriously injured and the helicopter sustained substantial damage to the tailboom and main rotor blades. The helicopter was registered to Dubois Aviation Inc and operated by the pilot as a 14 Code of Federal Regulations Part 91 solo instructional flight. Visual meteorological conditions prevailed and no flight plan was filed. The flight originated from CNO at about 1005.

The airline transport pilot and his passenger were not injured. The helicopter was registered to a private individual and operated by the pilot under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed and no flight plan was filed for the cross-country flight. The flight originated from White Sulphur Springs, Montana, about 1915, with a destination of Sutherlin Lake.
