NTSB Final Report Released: Page, AZ

The commercial pilot was conducting a personal flight in the helicopter to practice operating in the traffic pattern and performing landings and hovers. When the helicopter was transitioning from a hover to forward flight, the nose suddenly pitched down more than expected. The pilot immediately applied aft cyclic but was unable to arrest the downward pitch. The helicopter subsequently impacted the ground and came to rest on its side. The passenger reported that first the skids and then the rotor blades contacted the ground.

NTSB Preliminary Report Released: Clanton, AL

On November 16, 2018, about 1130 central standard time, a Bell OH-58C, N510CP, struck powerlines and came to rest in the Coosa River, near Clanton, Alabama. The helicopter was owned and operated by the Columbus Georgia Police Department. The commercial pilot and student pilot passenger were fatally injured. Visual meteorological conditions prevailed, and no flight plan was filed. The positioning flight was conducted under the provisions of Title 14 Code of Federal Regulations Part 91 and was destined for Chilton County Airport (02A), Clanton, Alabama. The flight departed Columbus Airport (CSG), Columbus, Georgia, about 1025 eastern standard time and stopped at Auburn University Regional Airport (AUO), Auburn, Alabama, to pick up the passenger before continuing to 02A.

NTSB Final Report Released: Fort Glenn, AK

The commercial helicopter pilot was conducting an aerial mustering flight with cattle on a remote island. The pilot reported that, while in a hover, he heard "metallic grinding noises" from behind his position in the front right seat; however, the helicopter was "stable with engine power," the rotor rpm was within normal limitations, and the had "normal responding controls and tail rotor authority." The pilot immediately landed the helicopter on a sandbar, completed the shutdown process, and exited from the helicopter without further incident. While conducting a postflight inspection, the pilot observed that the tail rotor gearbox had separated from the tailboom mounting assembly and was lying on the ground.

NTSB Final Report Released: San Juan, PR

The commercial pilot was conducting helicopter operations to support electrical workers on utility towers. The support included lifting ladders and other equipment with an open-end grapple and lifting ladders with electrical workers (linemen) on them with an A-frame attachment. At the time of the accident, the helicopter was attempting to lift a ladder with a lineman on it using a grapple. The ladder became bound, and the helicopter pilot descended to lower the ladder back on the static arm. The ladder became free of both the grapple and static arm, which resulted in the ladder falling to the ground with the lineman on it, causing a serious injury to the lineman.

Serbia receives first out of nine H145Ms

Donauwörth, The Serbian Minister of Defence Aleksandar Vulin visited the Airbus Helicopters Donauwörth site to accept the first H145M for the Serbian Air Force. In December, two H145Ms will be delivered to the Serbian Ministry of Interior. Altogether, Serbia will receive nine H145Ms, earmarked for the Air Force and for the Ministry of Interior. Four of the Air Force’s aircraft will be equipped with the HForce weapon management system.

Which U.S. Helicopter Industries Have the Best and the Worst Accident Records?

WASHINGTON DC – The U.S. Helicopter Safety Team (www.USHST.org) looked at 10 years of helicopter operations data from Jan. 2009 through 2018 and compared the share of flight hours of every industry type with the percentage share of total accidents and fatal accidents. During this period, U.S. civil helicopters flew more than 31 million flight hours and experienced 1,298 total accidents and 209 fatal accidents. Comparing the share of flight hours with the share of accidents within each industry area, the USHST developed these lists showing which industry areas have the best and worst records for total accidents and fatal accidents.

NTSB Final Report Released: Venice, LA

He reported that, while en route, he detected an in-flight vibration and made a precautionary landing on an oil platform. As he was shutting down the engine, the vibration increased, and he initiated an emergency shutdown using the rotor brake.

NTSB Preliminary Report Released: Kaneohe, HI

On October 22, 2018, about 1710 Hawaiian standard time, a Robinson R44 helicopter, N4132H, was substantially damaged during a hard landing on the Kaneohe Sandbar, a large area of sand covered by shallow water, located in Kaneohe Bay, near Kaneohe, Hawaii. The commercial pilot and two passengers sustained serious injuries. The helicopter was registered to Higgins Leasing Inc. and operated by Novictor Aviation under the provisions of Title 14 Code of Federal Regulations Part 91 as an air tour flight. Visual meteorological conditions prevailed for the local flight, and a company flight plan was filed. The helicopter originated from Honolulu International Airport, Honolulu, Hawaii at 1647, and was planned to complete a 45-minute circle island route flight.
