NTSB Preliminary Report: Brainerd, MN

On June 28, 2019, about 0041 central daylight time, an Agusta Spa A109S helicopter, N11NM, impacted terrain while landing at Brainerd Lakes Regional Airport (BRD), Brainerd Lakes, Minnesota. The pilot and flight nurse were fatally injured, the flight paramedic was seriously injured, and the helicopter sustained substantial damage. The helicopter was registered to and operated by North Memorial Healthcare under the provisions of Title 14 Code of Federal Regulations Part 135. Instrument meteorological conditions prevailed at BRD for the instrument flight rules (IFR) flight, which departed from North Memorial Heliport (MY77), Robbinsdale, Minnesota, about 2348.

NTSB Preliminary Report: Parsons, WV

On June 29, 2019, about 1140 eastern daylight time, a Robinson R44 helicopter, N883TY, was substantially damage when it collided with a telephone line and then a river near Parsons, West Virginia. The private pilot and one passenger sustained minor injuries, and a second passenger sustained serious injuries. The helicopter was registered to and operated by Ali Co. under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed at the time of the accident and no flight plan was filed for the local flight that originated about 15 minutes earlier from Elgon, West Virginia.

NTSB Final Report: Orlando, FL

The commercial pilot was conducting a local sightseeing flight in a helicopter with two passengers onboard. He reported that, while he was flying southbound and descending from 1,000 to 600 ft, he heard a "pop" sound and noted a corresponding right yaw, followed by vibration and engine roughness. The pilot looked at the instruments, and the main rotor tachometer was indicating that the rpm had dropped to "0" and that the engine tachometer was indicating between 60% and 70%. The pilot immediately initiated an autorotation to a median, and the helicopter landed hard. The helicopter began rolling forward, and in response, the pilot applied aft cyclic control input, which was contrary to the instructions in the Pilot's Operating Handbook, which contained a caution in the emergency procedures section related to power failures that stated to "not apply aft cyclic during touchdown…to prevent possible blade strike to the" tailboom. The pilot's improper control input resulted in the main rotor blades contacting and substantially damaging the tailboom.

NTSB Final Report: Myrtle Beach, SC

The commercial pilot and two passengers departed in the helicopter for a commercial air tour flight. The pilot reported that, shortly after takeoff, the helicopter started shaking like a "minor kick," which he thought might be due to a stuck engine valve. He immediately radioed the operator and requested that maintenance personnel meet the flight after the tour ended. He chose to continue the tour and attempted to maintain about 70 knots with the engine operating at 25 inches manifold pressure, which resulted in a descent rate of 300 ft per minute. When the helicopter was near the helipad location, company personnel reported seeing white smoke trailing from the helicopter, and the pilot noted that the alternator light was on. When the pilot started to slow the descent, the low rotor rpm horn sounded, and the engine rpm spiked. After realizing the helicopter would not be able to reach the helipad, the pilot turned the helicopter parallel to the slope of an adjacent field and fully pulled the collective pitch to cushion the landing, but the helicopter landed hard.

The commercial pilot was fatally injured. The flight departed from the East 34th street heliport (6N5), New York, New York, about 1330 and was destined for Linden Airport (LDJ), Linden, New Jersey.

The high altitude flight campaign of the new H145 starts in Chile
