Helicopter Industry Implementing Key Safety Practices More Often, Per Latest IHSF Global Survey

The International Helicopter Safety Foundation (www.IHSF.aero) has compiled data from its fourth annual survey of civil helicopter operators located in key regions around the world in order to gain a broader understanding of where safety initiatives are being implemented. Participation in the survey increased by 76 percent compared to a year ago. IHSF received nearly 1,300 responses to the survey from 112 different countries.

NTSB Preliminary Report: Talking Rock, GA

On March 5, 2019, about 1130 eastern standard time, an MD Helicopters (Hughes) 369D, N89ZC, was substantially damaged when it impacted trees and terrain near Talking Rock, Georgia. The commercial pilot was fatally injured. The helicopter was operated by Rotor Blade LLC as a Title 14 Code of Federal Regulations Part 133 external load flight. Visual meteorological conditions prevailed and no flight plan was filed for the local flight, which originated from a nearby field.

NTSB Preliminary Report: Union Center, SD

On March 3, 2019, about 0013 mountain standard time, a Bell 407 helicopter, N450AM, was substantially damaged on the ground near Union Center, South Dakota. The pilot and one crew member were not injured. One crew member sustained minor injuries. The helicopter sustained substantial damage. The flight was conducted under the provisions of 14 Code of Federal Regulations Part 135. Visual meteorological conditions prevailed and it is unknown if a Federal Aviation Administration (FAA) flight plan had been filed for the flight. The helicopter had arrived at the landing zone at Union Center to pick up a patient.

NTSB Final Report: Palm Bay, FL

The helicopter flight crew, consisting of a pilot, copilot, and maintenance crewmember, was performing 20-knot rearward flight about 200 ft above ground level as part of a post-maintenance functional check flight (FCF). While the pilot was flying and recovering from the first rearward flight maneuver, unusual sounds were heard, which the flight crew identified as a compressor stall. The pilot then told the maintenance crewmember that they were returning to their home airport; however, after discussing compressor stalls and engine exhaust gas temperatures with the copilot, the pilot changed his mind and told the maintenance crewmember that they were going to try the maneuver again in a different direction relative to the wind (with the wind off the nose). While the pilot was recovering from the second rearward flight maneuver, there was a change in background noise, which the maintenance crewmember identified as a compressor stall. About 2 seconds later, there was another change in background noise, consistent with a decay in drivetrain rpm, which was followed by the helicopter descending and impacting the ground.

A Step Backwards in 2018 for Helicopter Accidents in Key Global Regions

After gathering accident information from countries around the world, the International Helicopter Safety Foundation (www.IHSF.aero) is seeing a temporary setback for the industry’s objective of safer civil helicopter operations. Preliminary 2018 data from 50 countries show a year-over-year increase in fatal accidents and virtually no change in non-fatal accidents. However, the long-term trend remains positive. Fatal accidents during the three-year period of 2016-18 are down 19 percent compared to 2013-15 and non-fatal accidents are down 25 percent when comparing the past three years to the previous three years.

Mixed Results for U.S. Helicopter Accident Rates During 2018

WASHINGTON DC – Accident rates for the U.S. civil helicopter industry improved only slightly during 2018, but remained below accident totals from four and five years ago. Preliminary data shows that the 2018 accident rate was 3.62 per 100,000 flight hours, compared to an accident rate in 2017 of 3.70. There were 121 total accidents in 2018 compared to 123 accidents in 2017. Compared to five years ago, the accident rate has been cut 27 percent. On a less positive note, however, the fatal accident rate rose slightly year-over-year, with 24 fatal accidents occurring in 2018 and 20 fatal accidents in 2017.
