NTSB Preliminary Report Released: Fulton, MO

On October 17, 2018 about 1421 central daylight time, a Robinson R22B helicopter, N923SH, registered to Spitzer Helicopter LLC, Woodland Hills, California, and operated by Helisat LLC, Moscow Mills, Missouri, was destroyed when it impacted terrain near Fulton, Missouri. The commercial rated airplane pilot, who was the sole occupant, sustained fatal injuries. Day visual meteorological conditions prevailed in the area and a flight plan was not filed. The solo cross-country instructional flight was being conducted under the provisions of Federal Code of Regulations Part 91. The flight originated from a private airfield near Moscow Mills, Missouri about 1345 and was enroute to Columbia, Missouri (COU).

NTSB Final Report Released: Yakutat, AK

The helicopter pilot reported that, during startup on an off-airport landing pad, the helicopter was facing into the wind, and the airspeed indicator was showing gusting wind conditions. As the blades began to turn, he felt a strong wind gust and heard a loud bang almost simultaneously, and the helicopter "lurched" to the right about 5°. He immediately aborted the start procedure and applied the rotor brake.

NTSB Preliminary Report Released: Salinas, CA

On October 6, 2018, at 1437 Pacific daylight time, a Robinson Helicopter Company R44 II, N611TH, made a forced landing to an open plowed field following a loss of engine power on take off from Salinas Municipal Airport (SNS), Salinas, California. The pilot/owner operated the helicopter under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight. The pilot, the sole occupant, was not injured. The helicopter sustained substantial damage to the tail boom. Visual meteorological conditions prevailed for the local area flight, and no flight plan had been filed. The flight was originating at the time of the accident.
