NTSB Preliminary Report Released: Custer, SD

On August 2, 2018, about 0930 mountain daylight time, a Bell 47G 3B helicopter, N347BH, experienced an engine power anomaly during attempted landing at Crazy Horse Heliport (0SD9), Custer, South Dakota. On approach, the pilot noticed the RPMs had dropped below normal operating range. The pilot increased throttle but was unable to recover RPMs within normal range and the helicopter descended toward the ground. The pilot executed a forced landing in a grassy area on the side of a highway. The helicopter rolled to the right after landing and sustained substantial damage to the main rotor. The commercial-rated pilot and one passenger were not injured. Another passenger received minor injuries. The airplane was registered to Dakota Rotors and operated by Black Hills Helicopters under the provisions of Title 14 Code of Federal Regulations Part 91 as an instructional flight. Visual meteorological conditions prevailed for the flight, and no flight plan was filed.

NTSB Final Report Released: Prescott, AZ

The flight instructor and student pilot were practicing straight-in autorotations in the helicopter. During the autorotative descent, about 400 ft above ground level with the student at the controls, the instructor noticed a high rate of descent and a decay in main rotor rpm. The instructor told the student that he had the controls and started applying collective and throttle to recover rpm. He felt the collective go up and again told the student that he had the controls. The instructor was unable to arrest the descent before the helicopter landed hard and rolled over. The instructor reported that there were no pre-impact mechanical malfunctions or failures with the airframe or engine that would have precluded normal operation. He further stated that the accident could have been prevented if he had stayed ahead of his student and the helicopter.

On August 17, 2018, about 1050 Pacific daylight time, a Hughes 369D helicopter, N105JL, was substantially damaged following a hard landing during a practice autorotation at Riverside Municipal Airport (RAL), Riverside, California. The certified flight instructor received minor injuries and the pilot receiving instruction was seriously injured. The helicopter was registered to Helicopters Unlimited, La Crescenta, California, and operated by Western Helicopters, Inc., Riverside, California, in accordance with Title 14 Code of Federal Regulations Part 91. Visual meteorological condition prevailed for the local instructional flight, which departed RAL about 1030, and no flight plan was filed.

Kurt Robinson Will Discuss IHST Initiatives at FAA Rotorcraft Conference

WASHINGTON DC – Robinson Helicopter Company President and CEO Kurt Robinson will represent the International Helicopter Safety Team (www.IHST.org) in Texas next month when he addresses the helicopter industry at the FAA’s 2018 International Rotorcraft Safety Conference. The three-day workshop event takes place in Hurst, TX, on Oct. 23-25. The free conference (www.faahelisafety.org) gathers together rotorcraft pilots, mechanics, small company owners, industry executives, helicopter operators, students and government regulators from the United States and abroad. Robinson will offer a report on the IHST’s progress and will discuss accident trends, results of the global safety survey, and the IHST’s “Go Local” safety culture campaign. The “Go Local” program targets pilot decision-making and seeks volunteers to support the campaign in their local communities.

NTSB Final Report - Alexandria, MN

The pilot and two medical crew members were conducting a night instrument flight rules cross country flight to pick up a patient. During the instrument approach to the destination airport, the weather conditions deteriorated. The pilot was using the helicopter's autopilot to fly the GPS approach to the airport, and the pilot and the medical crew reported normal helicopter operations. Upon reaching the GPS approach minimum descent altitude, the pilot was unable to see the airport and executed a go-around. The pilot reported that, after initiating the go around, he attempted to counteract, with right cyclic input, an un-commanded sharp left 45° bank . Recorded flight data revealed that the helicopter climbed and made a progressive right bank that reached 50°. The helicopter descended as the right bank continued, and the airspeed increased until the helicopter impacted treetops. The helicopter then impacted terrain on it's right side and came to rest near a group of trees.

The systems include multiple EP-8100® image generators, 4K ultra-high resolution, liquid crystal over silicon (LCoS) laser-illuminated projectors, a 240 degree by 80 degree motion-compatible dome display, night vision goggle capability and additional channels for camera systems, vertical reference window, cargo mirrors and an Enhanced Vision System (EVS). NTSB Report Released: Basin City, WA

On August 7, 2018, about 1000 Pacific daylight time, a Bell 206B helicopter, N607RA, struck the ground during an agricultural spray run near Basin City Washington. The commercial pilot received minor injuries, and the helicopter was destroyed by a post-impact fire. The helicopter was registered to Ranger Aviation Leasing, and operated by Northwind Aviation as a Title 14 Code of Federal Regulations Part 137 aerial application flight. Visual meteorological conditions existed at the time of the accident. The flight originated from a landing zone near the accident site.
