NTSB Report Released: Glendale, AZ

On August 22, 2017, about 0904 mountain standard time, an Enstrom F-28F helicopter, N52PD, experienced smoke emanating from the engine cowling area while the flight instructor and student pilot were practicing hovering auto-rotations at the Glendale Municipal Airport (GEU) Glendale, Arizona. The helicopter had just touched down and therefore an immediate engine shutdown was accomplished. The flight instructor and student pilot were not injured. The helicopter was not damaged. The helicopter was registered to and operated by Airwest Aviation Academy LLC as a 14 Code of Federal Regulations Part 91 instructional flight. Visual meteorological conditions prevailed, and no flight plan had been filed. The local flight departed GEU about 0750.

NTSB Report Released: Tujunga, CA

On September 2, 2017, about 0948 Pacific daylight time, a Leonardo (formerly AugustaWestland Philadelphia) AW139 helicopter, N304FD, sustained substantial damage after impacting trees while conducting aerial firefighting operations in a residential area in Tujunga, California. The pilot and crewmember sustained no injuries. The helicopter was registered to the City of Los Angeles and was operated by the Los Angeles Fire Department as a public use visual flight rules aerial firefighting flight, operating as Fire 4. Visual meteorological conditions with moderate smoke were present in the area at the time of the accident and flight following procedures were utilized by the operator. The helicopter departed from the Van Nuys Airport, Van Nuys, California about 0815 to conduct the aerial firefighting operations for the day.

NTSB Report Released: Hertford, NC

On September 8, 2017, about 1120 eastern daylight time, a Eurocopter Deutschland GMBH MBB BK117-C2 helicopter, N146DU, was destroyed when it crashed on a wind turbine farm in Hertford, North Carolina. The commercial pilot, two flight nurses, and one patient were fatally injured. Day visual meteorological conditions prevailed at the time, and a company flight plan was filed for flight that departed the Sentara Albemarle Regional Medical Center Heliport (NC98) about 1108. The flight was destined for the Duke University North Heliport (NC92). The helicopter was operated by Air Methods Corporation under the provisions of 14 Code of Federal Regulations Part 135.
