NTSB Final Report Released: Alice, TX

Posted by jhadmin

The commercial pilot was practicing emergency maneuvers in the helicopter. He completed two successful power recovery practice autorotations; however, during the third, the pilot noted a vibration in the rotor during the landing flare. He reported that, when he initiated the power recovery, the engine power "wasn't there." The pilot then leveled the helicopter and used the collective to soften the landing. Upon contacting the ground, the skids separated from the helicopter and the helicopter rolled onto its side. Examination of the helicopter and engine at the accident site did not reveal any anomalies that would have precluded normal operation and the reason for the loss of engine power could not be determined.

NTSB Final Report Released: Skagway, AK

Posted by jhadmin

The non-instrument-rated commercial pilot was making a visual flight rules internal-cargo company flight in the helicopter. He was returning to base in the helicopter after dropping off 1 employee and 12 dogs at a remote dog camp situated on a glacier surrounded by mountainous terrain. The pilot had previously completed 5 of the day's 7 planned roundtrip flights from the base to the dog camp. According to the dog camp manager, the weather was deteriorating with snow and wind increasing when the pilot departed on the accident flight. The dog camp manager's observations and radar data indicated that the pilot attempted to depart via the normal route to the south but turned around. He likely encountered low visibility conditions and then attempted several departures by routes to the north of the dog camp. About 8 minutes after departure, the helicopter impacted snow-covered mountainous terrain about 2 miles northeast of the dog camp.

NTSB Final Report Released: Mountain City, TN

Posted by jhadmin

During the initial climb, about 125 ft above ground level, the commercial pilot of the helicopter heard a loud noise that was accompanied by a left yaw, rapidly increasing measured gas temperature, and decreasing power turbine rpm. During the subsequent forced landing, the helicopter landed hard, which spread the skids and resulted in substantial damage. Examination of the engine revealed a partial separation of the compressor diffuser, which allowed intake air to leak before combustion. Metallurgical examination of the compressor diffuser revealed that a portion of the aft plate had separated near a braze joint; however, manufacturer specifications indicated that the aft plate should have been a single component, not two components joined together by a brazing process.

NTSB Final Report Released: Gunnison, UT

Posted by jhadmin

The commercial pilot was nearing the end of his second agricultural application pass; about the time that he turned off the spray system, he heard a "pop" and felt a "thump" that shook the helicopter. After he observed chemical spray in the air and along the side of the helicopter, he pushed the "kill" button for the spray pump. As he was doing so, the engine rpm dropped, and the pilot heard the engine "sputtering." The pilot lowered the collective to re-establish main rotor rpm; the engine subsequently experienced a total loss of power. He entered an autorotation between 50 and 75 ft above the ground, and the helicopter landed hard.
