NTSB Final Report: Panama City, FL

Posted by jhadmin

The float-equipped helicopter was in cruise flight at an airspeed of 80 knots and an altitude of 800 ft mean sea level when it slowly started losing airspeed. The commercial pilot responded by pushing the cyclic control forward, but the airspeed continued to decrease, and the helicopter began to lose altitude. The pilot continued to push the cyclic forward until it contacted the control stop; he then realized that he had no cyclic control authority. The helicopter descended with no forward airspeed until it impacted the water. The helicopter floated briefly until waves struck its side and it rolled inverted. Postaccident examination of the helicopter revealed that the main rotor blades were deformed, the fuselage was substantially damaged, and the tail boom was partially separated. Further, the ropes used to tie down the helicopter’s main rotor blades were found wrapped around the rotor head swashplate and pitch control rods.

NTSB Final Report: New Castle, IN

Posted by jhadmin

According to the helicopter pilot, during an agricultural flight, he landed on the tender truck platform. The ground crewman connected a hose to fill the hopper, but he did not provide positive communication to the pilot regarding task completion. Consequently, "due to crew miscommunication," the pilot took off with the hose still connected to the helicopter. The tail rotor struck the hose, and the pilot lost tail rotor effectiveness. The helicopter entered an uncontrollable right spin, and the pilot lowered the collective. The helicopter descended, struck the ground, and then rolled over.

NTSB Final Report: Rulo, NE

Posted by jhadmin

According to the helicopter pilot, he was performing a night agricultural flight. During takeoff from a platform located on top of the tender, his collective application was too aggressive, and during the ascent, the engine sputtered. The engine rpm and main rotor rpm decreased, and he tried to regain the rpm by lowering the collective. The helicopter was moving forward and descending, so he applied aft cyclic to arrest the forward movement, and he attempted to land. The left skid touched down in the soft dirt, and the pilot applied aft cyclic to prevent a nose-over, but the main rotor blades struck the tailboom. The helicopter rolled forward, and its nose spun left before the helicopter came to rest on its left side.
