NTSB Report Released: Rockport, WA

On September 12, 2016, about 1345 Pacific daylight time, a high skid-equipped Bell 206BIII helicopter, N6181A, sustained substantial damage after landing in remote mountainous terrain on Mount Prophet in the North Cascades National Park, about 30 miles northwest of Rockport, Washington. The certificated commercial pilot and the sole passenger sustained no injury. The helicopter was registered to, Tony's Trucking, Inc. Darrington, Washington, and operated by Hi Line Helicopters, Inc., Darrington, as a day, visual flight rules (VFR) public use flight in contract with the National Park Service (NPS). Visual meteorological conditions prevailed at the time of the accident, and company flight following procedures were in effect. The flight originated from a helicopter landing zone near Diablo Lake about 8 miles south of the accident site, about 1330.

U.S. NAVY SELECTS LEONARDO'S OSPREY ELECTRONICALLY SCANNED RADAR FOR MQ-8C FIRE SCOUT

Leonardo-Finmeccanica’s Osprey AESA radar has been picked to serve as look-out on-board the US Navy’s newly-upgraded unmanned helicopter, the MQ-8C Fire Scout. The helicopter will be launched from the decks of US naval combat vessels to keep watch for distant threats. The contract will see Leonardo delivering an initial batch of 5 radars to the US Navy’s procurement organisation, the Naval Air Systems Command (NAVAIR), for testing and evaluation work. NAVAIR then has an option to buy a larger quantity of the radars for use in real operations. Leonardo has already built a number of Osprey radars so the primary task under this contract is integration with the MQ-8C Fire Scout in time for first production deliveries.

NTSB Report Released: Lino Lakes, MN

On October 6, 2016, about 1645 central daylight time, a Fairchild Hiller FH 1100 helicopter, N4035G, impacted terrain during an in-flight breakup and collision with terrain while maneuvering near Lino Lakes, Minnesota. The cockpit and cabin areas were consumed by a post-crash fire. The airline transport pilot and his passenger sustained fatal injuries. The helicopter was destroyed during the impact and fire. The helicopter was registered to Helicopter Connection LLC. and operated by the pilot under the provisions of 14 Code of Federal Regulations Part 91 as personal flight. Day visual meteorological conditions prevailed for the flight, which did not operate on a flight plan. The local flight originated from the Anoka County-Blaine Airport (ANE), near Minneapolis, Minnesota, about 1620.

On October 5, 2016, about 0915 mountain standard time, a Bell UH-1H, N175SF, rolled over during a precautionary landing near Gila Bend, Arizona. The helicopter was registered to and operated by Farm Ag Enterprises, as an aerial application flight under the provisions of 14 Code of Federal Regulations (CFR) Part 137. The commercial pilot was not injured, and the helicopter sustained substantial damage. The local flight departed a private airstrip about two hours prior. Visual meteorological conditions prevailed and no flight plan had been filed.
