NTSB Preliminary Report: Viroqa, WI

On August 19, 2019, about 1834 Central daylight time, an experimental light sport, Sport Copter Vortex gyrocopter, N634SC, impacted a corn field about .31 miles west of the departure end of runway 29 at Viroqua Municipal Airport (Y51) Viroqua, Wisconsin. The gyrocopter was owned and operated by the pilot as a visual flight rules (VFR) flight under the provisions of 14 Code of Federal Regulations Part 91 when the accident occurred. The non-certificated pilot was fatally injured and the gyrocopter was destroyed by the impact and post-crash fire. Visual meteorological conditions prevailed and no flight plan had been filed. The flight departed Y51 about 1830.

NTSB Preliminary Report Released: Odanah, WI

On October 29, 2018, about 1345 central standard time, an Airbus (Eurocopter) AS350B3 helicopter, N910S, impacted terrain near Odanah, Wisconsin. The commercial rated pilot, the sole occupant, was fatally injured and the helicopter was destroyed. The helicopter was registered to and operated by Enbridge Energy Company, Inc, as a 14 Code of Federal Regulations Part 91 observation fight. The flight departed Gogebic-Iron County Airport (IWD), Ironwood, Michigan about 1300 for the pipeline patrol, en route to Superior, Wisconsin.

NTSB Report: Oshkosh, WI

On June 9, 2018, at 1229 central daylight time, a Robinson R44 helicopter, N529DW, collided with cables during an initial climb after taking off from a field in Oshkosh, Wisconsin. The pilot was fatally injured. The helicopter was substantially damaged. The helicopter was registered to and operated by MF Helicopters LLC as a Title 14 Code of Federal Regulations Part 135 aerial photography flight. Visual meteorological conditions existed near the accident site at the time of the accident, and a flight plan had not been filed for the local flight. The helicopter was en route to Wittman Field (OSH), Oshkosh, Wisconsin.

NTSB Report Released: East Troy, WI

On May 4, 2018, about 1155 central daylight time, a Robinson R44, N444VH, experienced a loss of engine power during taxi at East Troy Municipal Airport (57C), East Troy, Wisconsin. The helicopter sustained substantial damage to the main rotor. The student pilot and instructor were not injured. The helicopter was registered to and operated by a corporation under the provisions of Title 14 Code of Federal Regulations Part 91 as a training flight for Part 137 agricultural operations. Visual meteorological conditions prevailed for the flight, and no flight plan was filed.

NTSB Report Released: Hazelhurst, WI

On April 26, 2018, about 2250 central daylight time, a Eurocopter AS 350 B2 helicopter, N127LN, impacted trees and terrain during cruise flight near Hazelhurst, Wisconsin. The pilot and two crewmembers were fatally injured. The helicopter was destroyed during the impact. The helicopter was registered to and operated by Air Methods Corporation as a Title 14 Code of Federal Regulations Part 91 repositioning flight. Night visual meteorological conditions were reported in the area about the time of the accident, and the flight was operating on a company visual flight rules flight plan. The flight originated from the Dane County Regional Airport-Truax Field (MSN), near Madison, Wisconsin, about 2104 and was destined for the Howard Young Medical Center Heliport (60WI), near Woodruff, Wisconsin.

NTSB Report Released: Blair, WI

Blair, WIOn March 7, 2018, about 1400 central standard time, a Hughes 369D helicopter, transporting a powerline worker via a long line, was involved in an accident near Blair, Wisconsin, when the worker contacted the powerlines and received serious injuries. The helicopter was not damaged and the pilot received no injuries. The helicopter was registered to HLH Holdings, LLC, and was operated by High Line Helicopters, LLC, under the provisions of 14 Code of Federal Regulations Part 133 as a Rotorcraft External-Load Operation. Visual meteorological conditions prevailed for the flight, which was not on a flight plan. The local flight had just originated when the accident occurred.

NTSB Report Released: Sheboygan, WI

On March 4, 2018, about 2324 central standard time, a Messerschmitt-Bolkow-Blohm BK 117 B-2 helicopter, N1174U, operated by Air Methods, Englewood, Colorado, sustained substantial damage during a hard landing at Sheboygan County Memorial Airport (SBM), Sheboygan, Wisconsin. The commercial pilot, flight nurse, and flight paramedic were not injured. The flight was operated under the provisions of Title 14 Code of Federal Regulations Part 135 as a helicopter air ambulance flight. Night visual meteorological conditions prevailed at the time of the accident, and a company visual flight rules flight plan was filed for the flight that departed about 2257, from Fond du Lac, Wisconsin. The flight was destined for the HSHS St. Nicholas Hospital, Sheboygan, Wisconsin.
