NTSB Final Report Released: Harrisville, NY

The commercial pilot was conducting a cross-country flight in an experimental amateur-built helicopter that was equipped with an automobile engine. When the airplane was about 6 miles from the destination and at a cruise altitude of 500 ft above ground level, the engine sputtered once and then lost all power. The pilot performed an autorotation into a low-lying valley with short trees. During the landing, the main and tail rotors contacted the trees, resulting in substantial damage.

NTSB Preliminary Report Released: Beekmantown, NY

On October 30, 2018, at 1611 eastern daylight time, an Aerospatiale AS355, N372CA, was destroyed when it collided with a wooden utility pole and power lines near Beekmantown, New York. The airline transport pilot and a lineman were fatally injured. The pilot-rated passenger and another lineman sustained serious injuries. The helicopter was registered to and operated by Catalyst Aviation, LLC, Hammonton, New Jersey, and was being operated under the provisions of Title 14 Code of Federal Regulations Part 133 as a rotorcraft external load flight. Visual meteorological conditions prevailed, and no flight plan was filed for the flight that departed from a designated landing zone near Plattsburgh, New York, at 1602.

NTSB Report Released: Albion, NY

On April 24, 2018, about 1403 eastern daylight time, an experimental, amateur-built Mosquito XE helicopter, N911CY, was destroyed by a postimpact fire after a hard landing shortly after takeoff from Gaines Valley Aviation Airport (NY06), Albion, New York. The private pilot was seriously injured. The helicopter was owned and operated by the pilot under the provisions of Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed at the time and no flight plan was filed for the local flight.

NTSB Report Released: Harrisville, NY

On January 28, 2018, about 1530 eastern standard time, an experimental, amateur-built Vertical Aviation Technologies 260L Hummingbird helicopter, N4045, operated by the commercial pilot, was substantially damaged during a forced landing, following a total loss of engine power during cruise flight near Harrisville, New York. The commercial pilot was not injured. The personal flight was conducted under the provisions of Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed and no flight plan was filed for the flight that originated from Potsdam Municipal Airport (PTD), Potsdam, New York, about 1500. The flight was destined to pilot's private residence in Harrisville, New York.
