NTSB Preliminary Report: Omaha, NE

On April 16, 2019, at 0730 central daylight time, a Bell OH-58A helicopter, N109PD, impacted terrain during an autorotation following a partial loss of engine power near Omaha, Nebraska. The pilot and co-pilot were not injured, and the helicopter sustained substantial damage to the tail boom. The helicopter was registered to and operated by the Omaha Police Department as a public aircraft operations flight under Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed in the area at the time of the accident. The flight originated from the North Omaha Airport (3N0), Omaha, Nebraska, at the time of the accident and was destined for Blair, Nebraska.

NTSB Final Report: Rulo, NE

According to the helicopter pilot, he was performing a night agricultural flight. During takeoff from a platform located on top of the tender, his collective application was too aggressive, and during the ascent, the engine sputtered. The engine rpm and main rotor rpm decreased, and he tried to regain the rpm by lowering the collective. The helicopter was moving forward and descending, so he applied aft cyclic to arrest the forward movement, and he attempted to land. The left skid touched down in the soft dirt, and the pilot applied aft cyclic to prevent a nose-over, but the main rotor blades struck the tailboom. The helicopter rolled forward, and its nose spun left before the helicopter came to rest on its left side.

NTSB Preliminary Report: Blair, NE

On July 27, 2019, at 1628 central daylight time, a Robinson R44 II helicopter, N7508Z, impacted terrain following a reported system malfunction near Blair, Nebraska. The commercial pilot sustained minor injuries, and the helicopter sustained substantial damage to the main rotor blades and tailboom. The helicopter was registered to and operated by NE IA Helicopters, LLC, Tekamah, Nebraska, as a Title 14 Code of Federal Regulations Part 137 aerial application flight. Day visual meteorological conditions prevailed at the time of the accident, and a flight plan was not filed. The local flight departed an off-airport location for aerial application operations at an unknown time.
