NTSB Final Report: Overton, NV

The flight instructor reported that he told the pilot receiving instruction in the helicopter to shoot an approach to a sandbar in a riverbed. The pilot approached to the south, and the wind was between about 20 to 25 knots from the south. About 20 ft above ground level, the instructor took the flight controls due to a steep and fast approach and initiated a go-around. He performed a climbing left turn to the north, and subsequently the helicopter descended, and he was unable to maneuver it into the wind. The helicopter landed hard, bounced, and yawed right. The helicopter contacted the ground again and rolled onto its left side. The main rotor blades struck the ground, and the tailboom separated. The helicopter sustained substantial damage to the tailboom. The instructor reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

NTSB Final Report: King City, CA

The helicopter pilot reported that, while conducting an agricultural application flight, he planned to fly over the top of a power pole. He applied full-up collective to climb over the pole, but the helicopter struggled to gain altitude. He added that, after the helicopter cleared the pole, the main rotor speed reduced and the helicopter "quit flying," so he decided to conduct a run-on landing on the field. The helicopter landed hard, and the main rotor blades struck the tailboom. The helicopter sustained substantial damage to the tailboom. The pilot reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

Leonardo lands success in the French EMS market with its AW109 Trekker

NTSB Preliminary Report: Rochester, IN

NTSB Final Report: Alexandria, LA

The pilot reported that he had departed in the helicopter from a local field at night under visual flight rules conditions. He reported that the weather was good, that he climbed the helicopter to 1,400 ft mean sea level for the 4-mile flight, and that he began the descent about 1 mile from the destination. He reported that he had "noticed and was consumed w/Engine & Rotor RPM, in the Yellow." During the approach, the helicopter stopped moving forward and began moving backward. The helicopter landed hard in a level attitude on the grass. The helicopter sustained substantial damage to the main and tail rotor drive systems and the fuselage. The pilot reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

NTSB Final Report: Smoketown, PA

The flight instructor reported that he lifted off in the helicopter and ascended to between about 3 and 4 ft above the grassy field north of the runway to show the student pilot how to hover, and while having positive control of the collective and antitorque pedal, he gave the cyclic control to the student. While the instructor was "guarding" the cyclic, the student applied right cyclic. Subsequently, the helicopter descended, and the right skid contacted the ground. The instructor applied left cyclic, but the helicopter dynamically rolled over. The helicopter sustained substantial damage to the main rotor system and horizontal stabilizer. The instructor reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

NTSB Final Report: Lebanon, IN

The pilot reported that he conducted a low-reconnaissance flight in the helicopter over the intended landing site, which was just east of a building. Shortly after, he realized that the landing site had changed to west of the building. He maneuvered the helicopter to a hover over the new landing site about 75 ft above ground level and started a vertical descent. During the descent, a company mechanic seated in the left seat told the pilot of an obstacle left of the helicopter. He acknowledged seeing the obstacle and continued to descend. Shortly after touchdown, as the pilot reduced the throttle setting to idle, the main rotor blades struck a tractor trailer, the helicopter yawed, and the tail rotor blades struck the trailer. The helicopter sustained substantial damage to the main rotor and tail rotor systems. The director of operations reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.
