NTSB Final Report: Arlington, IN

On July 6, 2018, about 1720 eastern daylight time, a Bell 47G-2 helicopter, N96195, was substantially damaged when it impacted a corn field near Arlington, Indiana. A postimpact fire ensued. The commercial pilot was fatally injured. The helicopter was owned and operated by Central Indiana Ag Services, LLC as a Title 14 Code of Federal Regulations Part 137 aerial application flight. Visual meteorological conditions prevailed and no Federal Aviation Administration (FAA) flight plan had been filed for the flight. The local flight departed about 1655 from a loading platform at a farm located 1 mile northwest of the accident location.

HAI Statement on NTSB Hearing on 2019 Air Ambulance Crash in Ohio

(May 20, 2020) – Like many others in the rotorcraft industry, staff members from Helicopter Association International (HAI) viewed the May 19, 2020, National Transportation Safety Board (NTSB) board hearing on the cause of a January 2019 crash of a helicopter air ambulance (HAA) flight in Ohio. HAI appreciates the extraordinary efforts made by the investigative team in determining the myriad factors that led to the accident, which took the lives of three people who were on a mission to save lives. On July 6, 2018, about 1715 central daylight time, a Bell 206B helicopter, N224KM, was substantially damaged during a forced landing near Bentley, Illinois. The pilot was not injured. The helicopter was registered to and operated by Woodley Aerial Leasing LLC under the provisions of Title 14 Code of Federal Regulations Part 137 as an aerial application flight. Day visual meteorological conditions prevailed for the local flight, which departed about 1705 from a load truck near Bentley, Illinois.

Appareo Releases Major Upgrade to Web-Based Flight Data Analysis Software

FARGO, North Dakota (May 18, 2020) — Appareo today released a major architectural upgrade to Appareo EnVision, the company’s analysis software system for Flight Data Monitoring (FDM) or Flight Operations Quality Assurance (FOQA) programs. Appareo EnVision is a web-based application that allows fleet operators to access past and current flight data, generate detailed reports, and trend the safety improvements in their operations. Prior to this release, the software accepted flight data captured by Appareo’s Vision 1000 cockpit recorder. With this architectural rewrite, the Appareo EnVision web app can now accept flight data from other devices, including other Appareo flight data recorders and even third-party sources.

NTSB Preliminary Report: Clewiston, FL

On May 3, 2020, about 1315 eastern daylight time, an Airbus Helicopters AS350 B3 helicopter, N1WT, made an emergency landing at Airglades Airport (2IS) in Clewiston, Florida. The pilot reported an inflight hydraulics anomaly, declared an emergency, and landed at Airglades Airport. The pilot reported no injuries and the helicopter was not substantially damaged. A postlanding inspection of the helicopter found the hydraulic pump belt had failed and the hydraulic pump was seized. The flight was conducted under 14 Code of Federal Regulations Part 91 that began at Fort Lauderdale Executive Airport (FXE), landed at Palm Beach County Park Airport (LNA), and departed LNA for 2IS.

NTSB Final Report: Luling, LA

On May 20, 2018, at 1049 central daylight time, an MD Helicopters, Inc. 369D helicopter, N362JS, was destroyed when it collided with a power line structure and impacted terrain while performing an aerial repair near Luling, Louisiana. The commercial pilot sustained minor injuries, one crewmember sustained serious injuries, and a second crewmember sustained fatal injuries. The helicopter was registered to and operated by Air2 LLC as a Title 14 Code of Federal Regulations Part 133 external load operation. NTSB Preliminary Report: Camp Dwyer, Afghanistan

Medical Mounting for Hamilton T1 Ventilator in Airbus H145

After replacing the old ventilators with the Hamilton T1 in the H145 helicopters, a new mount had to be developed because the existing one was not compatible with the newly acquired device. It was crucial for the customer to retain the existing assembly interfaces in the helicopter and on the patient transport stretcher.
