U.S. Helicopter Safety Team Names Nick Mayhew of L3Harris as Co-Chair

WASHINGTON DC – Nick Mayhew, General Manager of the L3Harris Arlington Training Center in Texas, has been named the new industry co-chairman of the United States Helicopter Safety Team (www.ushst.org). The USHST was formed in 2013 as a regional partner within the International Helicopter Safety Foundation (www.IHSF.aero) to lead the industry’s and government’s cooperative effort to promote safety and work to reduce fatal helicopter accidents. Mayhew succeeds Raj Helweg, chief pilot of Air Methods, who continues to serve on the USHST Steering Commiittee.

NTSB Preliminary Report: Sheffield, TX

The pilot reported that he was hovering near a fence line inspecting water and grass on his property. The helicopter drifted during the hover and hit the fence with the right skid. The helicopter impacted the terrain, resulting in substantial damage to main rotor blades, windshield, and door post.

NTSB Final Report: Alexandria, LA

The pilot reported that he had departed in the helicopter from a local field at night under visual flight rules conditions. He reported that the weather was good, that he climbed the helicopter to 1,400 ft mean sea level for the 4-mile flight, and that he began the descent about 1 mile from the destination. He reported that he had "noticed and was consumed w/Engine & Rotor RPM, in the Yellow." During the approach, the helicopter stopped moving forward and began moving backward. The helicopter landed hard in a level attitude on the grass. The helicopter sustained substantial damage to the main and tail rotor drive systems and the fuselage. The pilot reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

NTSB Final Report: Groveland, FL

The student pilot reported that, during landing, the helicopter experienced a hydraulic failure. He added that he "overreacted" by making multiple commands to the cyclic, which resulted in a tail strike, followed by a hard landing and roll-over. The helicopter caught fire, and the student egressed through the left door. The helicopter sustained substantial damage to the fuselage and tailboom. According to Federal Aviation Administration inspectors who interviewed the pilot immediately following the accident, the pilot stated that he overreacted and that he did not accomplish the helicopter manufacturer's Hydraulic Failure emergency procedure because he "didn't want to let go of the cyclic or the collective to turn off the hydraulic pressure switch." The hydraulic pump and the hydraulic lines were damaged during the postaccident fire; therefore, the reason for the reported hydraulic failure could not be determined.

Kopter SH09 helicopter test flights go ahead in Sicily

NTSB Preliminary Report: Colexico, CA

The commercial pilot reported that during an aerial application flight, he sprayed a row on the field and climbed to avoid wires on the south side of the field. He then made a right turn and began his descent back to the field. After he cleared the wires, he entered the field on the spray line and began raising the collective. He noticed that the torque gauge was responding, however, the rate of descent was not being halted and the helicopter subsequently impacted the ground and came to rest on its right side.
