NTSB Final Report: Luling, LA

On May 20, 2018, at 1049 central daylight time, an MD Helicopters, Inc. 369D helicopter, N362JS, was destroyed when it collided with a power line structure and impacted terrain while performing an aerial repair near Luling, Louisiana. The commercial pilot sustained minor injuries, one crewmember sustained serious injuries, and a second crewmember sustained fatal injuries. The helicopter was registered to and operated by Air2 LLC as a Title 14 Code of Federal Regulations Part 133 external load operation. Day visual meteorological conditions prevailed at the time of the accident, and a company flight plan was filed for the flight, which originated from Port of South Louisiana Executive Regional Airport (APS), Reserve, Louisiana, at 0936.

NTSB Preliminary Report: Camp Dwyer, Afghanistan

On 20 April, 2020, at approximately 0800 local time, a Sikorsky S-61N, N908CH, experienced a loss of control in flight and rolled on its side during an emergency landing at Camp Dwyer, Afghanistan. The three crew members onboard were seriously injured and the helicopter sustained substantial damage. The flight was operating under the provisions of 14 CFR Part 135 as a cargo flight under contract to the Department of Defense.

Medical Mounting for Hamilton T1 Ventilator in Airbus H145

After replacing the old ventilators with the Hamilton T1 in the H145 helicopters, a new mount had to be developed because the existing one was not compatible with the newly acquired device. It was crucial for the customer to retain the existing assembly interfaces in the helicopter and on the patient transport stretcher.

NTSB Final Report: Timmonsville, SC

On April 21, 2017, about 1632 eastern daylight time, a privately owned and operated Bell-Carson 47G Super C-4 helicopter, N865Z, was destroyed by impact forces and a postcrash fire near a private uncharted airfield in Timmonsville, South Carolina. The private pilot was fatally injured. The helicopter was privately owned and operated as a Title 14 Code of Federal Regulations Part 91 personal flight. Visual meteorological conditions prevailed at the time of the accident, and no flight plan was filed for the local flight. The flight originated from Huggins Memorial Airport (58J) in Timmonsville about 1628.
